Financial Policies

Dr. Johnson will provide a treatment plan for any diagnosed conditions of your mouth. Your treatment plan will include a breakdown of all applicable fees, and we will give you a cost estimate before treatment is started. We will also provide an estimate of your insurance benefit and charges.

Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. If special arrangements are needed, these options can be discussed with our front office team prior to receiving services. For our non-insured patients, we offer a cash courtesy discount of 5% if you would like to pay your services in full up front. This option can only be extended if paying by cash or check. Credit cards have a servicing fee which prohibits us from extending the courtesy discount for that type of payment.

As a courtesy, we will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account should insurance not pay. Your insurance is a contract with the insurance company and your employer. If your insurance does not cover what was expected, you will be responsible for the amount remaining. In addition, if during the course of treatment your benefits change, the financial responsible party is required to notify our staff. If benefits are lost for any reason during treatment the remaining balance of the account is owed by the responsible party.

Please understand there are always contractual limitations within the insurance plans that may not always be in your best interest. Dr. Johnson bases his clinical decisions on what he feels are in your best interest, not in what your insurance may or may not cover.
